 Oh, I know, I know. Everyone's just typing lists on their cell phones. Well, mostly. There was just something so comforting to me to design with these retro patterns and image. Plus, I did add a modern twist and make this a fillable PDF, so at least you have the option to fill it out on the computer and print. So if you're in a retro-ish mood, here you go. :)
